Low

The Low plan is a High Deductible Health Plan (HDHP) paired with a tax-advantaged Health Savings Account (HSA).  Members must meet the deductible first before the plan will begin sharing in the cost of covered services.  However, employees will pay less per paycheck for this coverage compared to the Middle and High plans.  

Medium

The Middle plan is a PPO plan with lower deductibles than the HDHP and copays that limit costs for certain services.  Enrollees in the Middle plan are ineligible to contribute to an HSA.  However, they are eligible to contribute to a Healthcare FSA. 

High

The High plan is a PPO plan with copays and the lowest deductibles but the highest employee contributions per paycheck.  Like in the Middle plan, enrollees in the High plan are ineligible to contribute to an HSA.  However, they are eligible to contribute to a Healthcare FSA. 

The ER isn’t your only option

Your primary care doctor is usually the best place to start when you need care, but you have other options for non-emergency care — even in the middle of the night.

Remember, going to the emergency room (ER) or calling 9-1-1 is always best when it’s an emergency.

HSA and FSA plans work differently and have different restrictions, so make sure you understand what is right for you.

Find a Doctor

  1. Go to anthem.com/find-doctor.
  2. Choose your search:
    -Search as a Member: Use your member ID card number or log in with a user name and password. Next click “Find Care”.
    -Search as Guest: Click Change State to Colorado (at top of page). In drop down menu select Medical for Type of Care, Colorado for State, Medical Networks for Type of Plan, Choose Anthem PPO for the Plan/Network.
  3. Filter your search by name, specialty, or location, then click “Search”. You can also search within a certain distance of your location.
